Pune: State Health Department has sketched out a procedure for specially-abled citizens to get a disability certificate from authorities in a simpler way. Beneficiaries can now visit www.swavlambancard.gov.in website and they can also visit government centres for detailed information.

In a recent General Resolution (GR), the State government stated that patients who are visually challenged, hearing impaired, physically challenged, etc and other multiple disabilities can avail a disability certificate.

According to the GR, district hospitals, government-run and municipal corporation-run hospitals can now give a disability certificate. The form can be filled only on the website www.swavlambancard.gov.in. Along with the form,  residence proof and two passport size photos are needed. According to the GR, the disability certificate will be displayed on the website as well.
